The Client

A Retail Healthcare Clinic providing common outpatient services to walk-in patients. The visits are performed by Nurse Practitioners or physicians that are not the primary care providers for the patients.

The Challenge

Today’s patient may see a variety of health care providers for specific needs or specific situations. The retail clinic offers a convenient, fast solution for a patient that has a common diagnosis such as strep, flu, etc. These services may be performed by a physician other than the primary causing the visit to be unknown to the primary or the patient’s home clinic. This information needs to get to the primary care provider in a timely and convenient manner.

The Retail Clinic was sending patient summaries to primary care physicians via US Mail and Fax. These summaries would only get entered into the EMR if the office staff took the time to manually enter the information from the hard copy.

The Solution

The solution created the ability to transmit patient profile information from a retail clinic EMR to the patient’s primary care provider EMR via the Continuity of Care Record (CCR). The project involved:

  • Determining and documenting a Retail Profile Implementation Guide for the CCR. This guide explained the expectations around the usage of the CCR message.
  • Establishing relationships with the necessary intermediaries and end parties interested in receiving the information. Willing parties needed to be determined and educated to be able to receive this electronic document.

What roles did LogiSolve perform for the client?

  • Developing and maturing the Retail Profile for the CCR standard that is used to transfer the information within.
  • Managing the timeline and project plans for parties that extended beyond our immediate client.
  • Providing analysis and technical resources to complete the necessary work.

The Results

The client can now communicate patient-specific information via a secure channel to the patient’s primary care physician. This information can be systematically consumed by the recipient’s EMR. The primary care physician can now be aware of any patient care information that was provided by an external provider in a timely manner.
